Things to bring to your appointment
In an effort to provide efficient service, we ask that you please complete your health history form and return it to our office by mail, email or fax before your scheduled appointment. 

Please bring your card and insurance information with you to each visit so that we can make sure our records stay current.

If your referring dentist has provided you with x-rays and referral forms, please bring those to your appointment. 



Pre-Authorizations
A request to your insurance company to pre-authorize dental treatment will be done before treatment is started. The pre-authorization will give an overview of the projected costs allowed by your particular insurance plan. Any treatment not pre-authorized by your insurance company will necessitate payment in full for treatment by the patient. Payment for unauthorized treatment must be paid when treatment is started. Any unpaid balance is due when treatment is completed.The estimated portion not covered by your insurance is due at the time the treatment is performed. Please be aware that we are only capable of estimating your portion and we will collect this at the time services are rendered. Once the insurance company has paid their portion of the charges, if there is a balance remaining, we will send you a statement.



Initial Consultation
Your consultation will include a review of your medical and dental history, review of referring Doctors findings, visual examination, charting, periodontal probing, diagnosis, and treatment recommendations.



Treatment Planning
includes a schedule of procedures and appointments designed to restore, step by step, your oral health. The plan contains the advantages, disadvantages, costs, alternatives, and sequence of treatment. We will present the recommended treatment to you, as well as to the insurance company, for approval.
